Heartburn: when should you get it checked?

Almost everyone knows it: a burning feeling behind the breastbone, acid regurgitation, especially after a heavy meal or when lying down. Occasional heartburn is normal. If it occurs regularly, it is called reflux disease, and that should be treated.

Why reflux should not be ignored

In reflux, stomach acid flows back into the oesophagus. Over time this can inflame the lining. In some people the lining changes (Barrett's oesophagus), which increases the risk of oesophageal cancer. That is why it is worth investigating chronic reflux and getting it well controlled.

Warning signs to take seriously

  • Difficulty swallowing or the feeling that food gets stuck
  • Unintentional weight loss
  • Vomiting, especially with blood
  • Black stools
  • Anaemia (iron deficiency)
  • Heartburn that does not improve despite medication
  • New heartburn from around age 50

In these cases a gastroscopy should be done promptly.

What helps day to day

Smaller meals, no late dinners, raising the head of the bed, losing weight, less alcohol and nicotine. Many patients also benefit from acid blockers, which however should not be taken unsupervised for years.

When to see a gastroenterologist?

When heartburn occurs more than twice a week, lasts longer than a few weeks or one of the warning signs appears. In the consultation we clarify whether a gastroscopy makes sense and how the reflux can be brought under control for good.
